Discussing end-of-life care planning is one of the most important yet challenging responsibilities social workers face. Join Kristy Case, MSW, LCSW, OSW-C, for this workshop on navigating these conversations with compassion, confidence, and clinical skill.
This 3-hour interactive workshop equips clinicians with the theoretical frameworks and practical tools necessary to facilitate meaningful, culturally sensitive Advanced Care Planning (ACP) conversations.
We will explore compassionate, real-world communication strategies to help patients voice their true values, navigate difficult family dynamics, and translate complex medical realities into clear care choices. Beyond the paperwork, we will take an honest look at our own relationship with loss. Through self-reflection and shared case studies, we will look at how our personal beliefs and experiences with grief show up in the room (countertransference), learning how to protect our own energetic boundaries and prevent burnout while showing up fully for our clients.
Learning Objectives
Participants will be able to:
- Utilize at least two evidence-based communication strategies to effectively initiate, guide, and document advanced care planning and end-of-life choices with patients and their designated proxies.
- Identify three ways our own unspoken biases, personal beliefs, or past grief regarding death can subtly impact our clinical boundaries and get in the way of a patient’s right to choose.
- Formulate clinical interventions to mediate conflict, address cultural variances, and align care decisions with the patient’s explicit self-determination when family members or healthcare proxies disagree on end-of-life interventions.

About the Instructor
Kristy Case, MSW, LCSW, OSW-C

Kristy Case is a licensed clinical social worker, certified oncology social worker and end-of-life doula with nearly two decades of experience specializing in the impact of cancer and complex medical conditions. In addition to working for New Jersey’s largest academic healthcare system, she is the founder and CEO of a private behavioral health practice, where she provides evidence-based psychotherapy, clinical supervision, and group facilitation. Kristy is a sought-after speaker and educator, regularly presenting at conferences as well as Universities on the intersection of mental health, trauma, and high-stress experiences. She brings a systems-level perspective to her work, drawing from neuroscience and trauma-informed care. With certifications in Yoga and Reiki, Kristy integrates mindfulness-based modalities into her work to improve outcomes and resilience in demanding personal and professional life experiences.
Cancellation Policy
Unfortunately, we cannot provide refunds for cancellations made seven working days or fewer before the event for any reason—or for no-shows. We can provide credit towards a future workshop up to 24 hours before the event. After that, no credit will be issued.
